  • Full-time Community Program Facilitator

    The Program Facilitator plays a vital role in organizing, coordinating, and delivering program activities, including the Endless Possibilities, Making Opportunities Matter (M.O.M.), and After Party curricula. This position requires an adaptable, proactive individual who can recruit participants, manage multiple assignments, and foster relationships with community stakeholders, schools, and families. The Program Facilitator will support youth development, parental engagement, and outreach efforts, ensuring alignment with the mission of Pregnant with Possibilities Resource Center (PPRC) to empower women, youth, and families.

    To be a successful facilitator, you should be driven to succeed, able to handle stress, and have excellent communication and interpersonal skills. You should be familiar with teaching or administrating curriculum to fidelity, must be organized and can work independently.

  • Full-time Care Coordinator

    The Care Coordinator at Pregnant with Possibilities Resource Center plays a crucial role in supporting pregnant and postpartum individuals through comprehensive case management and health education. This position involves providing tailored support, conducting monthly visits, identifying client needs, and addressing barriers to care. As a certified Community Health Worker (CHW), the Care Coordinator will educate clients on prenatal and postpartum health, facilitate referrals to healthcare and social services, and empower clients to achieve their health and family goals. The role also includes maintaining accurate documentation, coordinating community outreach, and promoting positive health outcomes through culturally competent and empathetic client interactions. This position requires a proactive, organized professional who values diversity and is committed to helping families thrive.
